Infantile acropustulosis in children

Characteristic

Children between the ages of 2 and 10 months become ill. Sometimes they are born with skin changes. Boys of the black race have a predisposition to this disease, but children of both sexes and any population can get sick. The reason is unknown.

The clinical picture

The changes on the skin are initially separate erythematous papules 1-2 mm in size, which become vesiculopustular within 24 hours, then become crusted and heal. The palmar and plantar surfaces are a favorite localization, on which a profuse lesion can develop. The number of white blood cells and the leukogram do not differ from the norm. Numerous neutrophils are found in smears of the contents of the bladder during Wright staining, sometimes eosinophils predominate in them. There are no microorganisms in the crops. Histologically, pustules are subcorneal formations with clear boundaries filled with segmented leukocytes and eosinophils (sometimes the latter are absent).

Differential diagnosis

The range of differential diagnosis includes transient pustular melanosis of newborns, toxic erythema, cutaneous candidiasis, staphylococcal pustulosis. In older children, the disease should be differentiated from scabies, dyshidrotic eczema, pustular psoriasis, subcorneal pustular dermatosis, and hand, foot, and oral diseases.

Treatment

Treatment is aimed at reducing discomfort. However, infantile acropustulosis is relatively resistant to the local effects of corticosteroids and oral administration of antihistamines.
